Instructions for Filling the Online Form

1. Eligibility

• Only those students are allowed who have received Gold Medal / Top Rank in the 2024 Health Awareness Exam.

2. Basic Details

• Enter your Full Name (as per school record).

• Enter your Father’s / Mother’s Name.

• Enter your Class & Section correctly.

• Mention your School Name & Address.

3. Contact Details

• Provide a valid Mobile Number (WhatsApp preferred).

• Enter a correct Email ID for communication.

4. Photo Upload

• Upload a recent photograph of the student.

• Gold Medalists must upload a photo wearing the medal with school background.

• If more than one student from the same school won, then upload a Group Photo with school background.

5. Submission

• After filling all details, check the information carefully.

• Click on Submit / Register Now button.

• After submission, you will receive a confirmation message on your registered mobile/email.

6. Exam Details

• The exam date, time, and login password will be shared soon on your registered contact details.
